Incident Scenario: Neglecting Admission Assessments for a Patient

At the time of the incident, she was employed as an LVN at a healthcare facility in Midland, Texas, and had been in that position for two (2) years and five (5) months.

On or about March 24, 2022, while employed as an LVN at a health care facility in Midland, Texas. LVN failed to perform and document an admission assessment for a patient. LVN's conduct created an incomplete medical record and was likely to injure the patient in that subsequent caregivers would not have complete information to base their decisions for future care.

Without consulting with an experienced nurse attorney, LVN responded to the above incident and stated that she received emails asking her to do the admission assessment, so she documented it when she returned to work. LVN added that the unit's policy regarding which assessment tool to use has been changing.

The Texas Board of Nursing gave the LVN enough time to defend the complaints filed against her.
