Once a charge from the Board is filed, that case becomes public record and available for the world to see. When this happens, this makes it very difficult to get a job that takes insurance. On or about November 20, 2016, while employed in Plano, the nurse allegedly falsely documented that she performed wound assessments and care when in fact she did not complete them.

In response to the incident, the LVN states that she did write a rough draft nursing note from what she had previously assessed when assisting with the patient’s dressing change. She states that she did believe she had saved the note as unfinished which would allow her to go back and edit the note with her current assessment findings.

The LVN states that she mistakenly saved the note as finished without completing wound care for the patient. She states that at the end of her shift she had completed and documented all of her other assessments, wound care, and medication administration for all the patients in the infirmary, as well as the other duties assigned to her by her charge nurse. She states that in her shift report to the oncoming infirmary nurse, she did report that she did not complete this patient’s dressing change.
