On or about April 2015, through May 2015, the LVN failed to completely document care and assessments, and failed to properly follow-up care and assessments, on a patient. Additionally, her conduct exposed the patient unnecessarily to a risk of experiencing unrecognized clinical complications.
On the same day, the LVN also documented assessments of a patient, in the medical record of another patient. Her conduct created an inaccurate medical record and was likely to injure the patient in that subsequent care givers would rely on his documentation to further care for the patient.
The LVN was then summoned by the Texas Board of Nursing. During the defense, she states that she accepts responsibility for errors in documentation.
Because of this incident, the Texas Board of Nursing then subjected the LVN and her license into disciplinary action.
