On or about May 8, 2017, through June 19, 2017, while employed as a Licensed Vocational Nurse in a medical facility in Fort Worth, the LVN submitted fraudulent Payroll Summary Worksheets, in that she logged OASIS assessments on days she did not complete assessments, logged ineligible tasks as assessments, logged the same OASIS assessment multiple times on different days, and logged OASIS assessments that were completed by other coders as her own. Her conduct created an inaccurate billing record and was likely to defraud and deceive the facility of money paid to the LVN for work she did not perform.
On the same dates, the LVN also misappropriated monies belonging to the facility, in that she billed for fraudulent assessments, resulting in her being paid for hours she did not work. Her conduct was likely to defraud and deceive the facility of money paid to the LVN for work she did not perform.
When summoned by the Texas Board of Nursing, the LVN denies all allegations. She states in one instance of administration, she realized later the system had not saved the administration and would not allow her to chart it once she realized the issue.
The Texas Board of Nursing then subjected the LVN and her license into disciplinary action.
