On or about March 4, 2004, the RN removed 2.0 mg of Dilaudid at 12:52 a.m. and, again, at 5:44 a.m. from the Pyxis system using a patient’s patient information. The RN documented that she administered the Dilaudid on March 3, 2004, instead of March 4, 2004.

A Pyxis system is an automated medication dispensing machine. In order to obtain medication from the Pyxis machine, a nurse must enter a password into the Pyxis system. The Pyxis machine stores information on all withdrawals.

The Texas Board of Nursing gave the RN the chance to defend herself. However, she was not able to provide a good defense for herself, especially when the drug screening results were brought up to her. Therefore, the Board placed her RN license to a disciplinary action instead.
