On or about October 19, 2016, through August 8, 2017, while employed as a Licensed Vocational Nurse (LVN) and Assistant Director of Nurses (ADON) in a medical facility in Fredericksburg, Texas, the LVN failed to ensure that a policy was in place to assess potential residents for risks associated with criminal history, including registered sex offenders.
